Nothing most of the time . Sometimes really tight spandex or latex underwear though if I’m in the mood.

Usually naked but sometimes for variety I like to wear briefs so I can feel the latex against my legs but not my junk. Sometimes I put a sheet over the balloon, too, so there's more friction and it's more like dry humping.
